Management Services
Case Management:
Our agency offers case management to individuals who are in need of assistance with coordinating medical care, mental health care, and/or daily living functions. This services includes continually assessing the client’s current situation, determining and resolving unmet needs, and helping attain client defined goals for improvement in medical, emotional and psycho-social status. This is done by the case manager through an interdisciplinary approach, working with other providers and community agencies to ensure continued access to this program, and other programs that may be of service to the individual.
Springing Power of Attorney:
In accordance with New Mexico law, our agency offers the services for Healthcare and/or Financial Powers of Attorney (POA). This POA would go into effect when an individual had been deemed incapable of decision making as noted in the document.
Money Management:
Our agency offers money management to individuals who are in need of assistance with budgeting, paying monthly bills, working with creditors to negotiate reasonable payment plans, and long term financial planning. This service includes assessing the client’s current situation, determining and resolving unmet needs and helping clients set and reach goals. A financial manager will also work with other community agencies to ensure access to other programs that may be of service to the individual.
Representative Payee Services:
A Representative Payee is an individual or agency that assists people with disabilities who receive Supplemental Security Income but are unable to manage those funds. As a Representative Payee, Necessity CMC assures that we are aware of the beneficiary's needs so it can be decided how their funds can be best used for the individual's personal care and well-being. An accounting is required on a yearly basis to the Social Security Administration to assure that all expenditures are made in the best interest of the recipient.
